Maximum Coverage period:

For age 85 and younger, coverage is available for up to 365days under one policy. For age 86adn over, coverage can be purchased to insure up to the first 180 days following the arrival date.

 

Additional $50,000 of coverage with the $100,000 option

When you purchase the $100,000 Aggregate Policy Limit, a bonus $50,000 of additional coverage is automatically included for insured Services incurred as a result of an injury.

 

$10,000 Accidental Death & Dismemberment

$10,000 Accidental Death and Dismement Covearage is automatically inculded for All applicants.

AGE MEANS THE APPLICANT'S AGE ON THE EFFECTIVE DATE OF THE POLICY.

 

Deductible Options (Up to age 85): For $0 deductible, apply 5% surcharge to premium. For $250 deductible, apply 10% discount to premium ($500 deductible applies for age 86 and over, this deductible cannot be waived or modified.)

 

Family Rates:  Charge 2X the oldest applicant's total premium and list all "family" Applicants on the application. A "Family" is defined as three or more of, parent(s) or legal guardian(s) and their unmarried children under age 21 who are visiting Canada with them and dependent on them for their sole means of support.

 

Mimum policy Premium is $25.00  Daily rates are per person per day

BENEFITS:
Maximum Benefits: $10,000; $15,000; $25,000; $50,000 and $100,000.
Hospital and medical expenses - Up to the amount of insurance purchased for emergency hospitalization (ward accommodation) or out-patient services.
Physician and Private duty registered nursing or licensed home care providers.
Medical Appliances - rental of crutches, hospital bed, splints, trusses, braces or other prosthetic devices, up to a maximum of $5,000.
Professional Medical Services referred by a physician - treatment provided by a licensed chiropodist, chiropractor, osteopath, physiotherapist or podiatrist up to a maximum of $1,000 per insured.
Diagnostic Treatment including X-rays, ultrasounds, and laboratory tests requested by a physician up to the policy limit while hospitalized for a period of 24 hours or more or up to a maximum of $1,000 when these services are provided on an outpatient basis.
Ambulance Transportation.
Prescription drugs or medicines up to $500 per insured maximum.
Funeral Expenses: In the event of death, up to $5,000 for preparing and transportation of the body to the country of origin or up to $1,500 for cremation and/or burial at the place of death.
Emergency Transportation: The cost up to the policy limit of air ambulance, one-way economy airfare or stretcher to transport you to your country of origin, and a medical attendant if required, when immediate medical consultation is required following a covered emergency sickness or injury.
Accidental Dental: Up to $1,000 for repair or replacement of whole or sound natural teeth damaged by an accidental blow to the face.
Dental Emergencies: Up to $300 (per 12 months) for the immediate relief of acute dental pain, other than provided under Accidental Dental.

EXCLUSIONS:
AGE AND PERIOD LIMITS:
Coverage is available under a policy for 365 days in total for ages up to 85. For those age 86 and older, coverage can be purchased to insure up to the first 180 days following the date of arrival in Canada.

DEDUCTIBLES: The policy has $50.00 deductible per claim. For age 86 or greater - $500 deductible per claim. $0 deductible option is also available for visitors up to age 85.

WAITING PERIOD:
- 72 hours if the policy is purchased within 30 days of your arrival in Canada.
- 7 days if the policy is purchased after 30 days of your arrival in Canada.
- No waiting period if the insurance is purchased prior to your arrival in Canada or the policy has been extended.

PRE-EXISTING CONDITIONS:
Any sickness, disease or bodily injury in the 180-day period immediately preceding the "Effective Date" of this Policy.

Note: Each time you extend or purchase another policy from the insurance company because you are staying in Canada longer, each new policy will have a new effective date and you will NOT be covered under the new policy for any sickness, disease or bodily injury which had manifested itself in the 180-day period immediately preceding the new effective date.

"Stable Chronic Condition" means:
- A condition which is under treatment and which has been controlled by diet or consistent use of medication prescribed by a physician, and there have been no new symptoms or change in symptoms; and/or there has been no hospitalization or change in treatment, medication or dosage in the 180 days prior to the effective date of this policy.
- If your physician has determined that your health condition has improved and changes, your treatment, medication or dosage due to your improved health condition, this does not constitute a change.
- A condition that existed more than 180 days prior to the effective date and which did not require continued treatment as determined by a physician during the 180 days prior to the effective date of this policy.
- “Stable Chronic Condition” option is automatically included for visitors under age 70. Applicants age 70 to 85 who wish to purchase this option must complete the medical declaration on the application to determine eligibility for coverage.

OTHER EXCLUSIONS: 
- Intentional self-injury, suicide or attempted suicide while sane or insane.  
- Sickness, disease or bodily injury in any way arising from or contributed to by the misuse of drugs or alcohol.  
- Accidents which occurred while "You" participated in professional sports, mountaineering, any kind of race or speed contest, underwater activities, gliding, parachuting or skydiving.  
- Pregnancy, miscarriage, childbirth or any complications thereof.  
- The provision of "Insured Services" to children 30 days of Age or younger.  
- War, invasion, terrorist acts.     
- "Your" obtaining medical or healthcare assessment or any form of report or document for the purposes of supporting an application to obtain immigrant status in Canada.  
- Mental and emotional disorders including but not limited to stress, anxiety, depression (reactive or otherwise) and/or conditions related thereto.  
- General health examinations, medical and health services, including prescription drugs or medicines, provided to monitor or maintain a "Stable Chronic Condition".

CLAIMS:
To make a claim, you will need to complete a claim form and attach the original medical bills, receipts and invoices (always make a copy for your record), your medical and/or hospital record (diagnosis, X-ray, lab tests confirming that the treatment was medically necessary), copy of police report (in case of a Motor Vehicle Accident) and mail it to:

Claim Assistance: In the event of claim or in the event of circumstances likely to result in a claim, please call the Claims Assistance Centre at: 1-866-228-6386 (toll-free within North America) from 8:30 AM to 8:00 PM Eastern Standard Time.

In the case of any disagreement with 21st Century Travel Insurance Limited claim decision you may choose to follow the Claim Complain Process:
- Request in writing, a Mandatory Claims Review Meeting with 21st Century Travel Insurance Limited. A Mandatory Claims Review Meeting will be scheduled within 30 days of receipt of your written request.
- Arbitration in accordance with the law governing arbitration proceedings in the Province in which your policy was issued in case of any disagreement after the Mandatory Claims Review Meeting. Legal action to recover a claim must start within 12 months of the date of bodily injury or the date on which you first received any emergency medical service.

REFUNDS:
No premium will be refunded if a claim has been paid or is pending under your insurance. A $25 administration fee will be charged if the policy is cancelled at any time.

SIDE TRIPS:
This insurance provides coverage for up to 30 days while traveling outside Canada as long as your side trip originates and terminates in Canada and you spend in excess of 50% of your time in Canada. You are not covered under this insurance policy in your country of origin.

What is the Stable Chronic Condition Option?
When this option is automatically included or purchased for the extra premium found in Table 2, the Pre-existing Condition Exclusion in the policy will not apply to Insured Services incurred after any applicable Waiting Period to respond to a Stable Chronic Condition (as defined in the policy).

Note that, even with this option included, conditions that do not fit the definition of Stable Chronic Condition are still subject to the regular exclusion (reproduced below).

Pre-existing Condition Exclusion:
21ST Century Travel Insurance Inc. will not reimburse you for Insured Services arising from any sickness, disease or bodily injury which had manifested itself in the 180-day period immediately preceding the Effective Date of this Policy.

Unexpected Emergency sickness, disease or bodily injury shall be considered to have manifested itself when:
- medical care, advice, investigation or treatment has been received, or
- drugs or medicines have been taken or prescribed to treat the sickness, disease or bodily injury, or
you have experienced symptoms which would cause a reasonable prudent person to seek diagnosis, care or treatment.
Under this exclusion, each time you purchase another Policy from the insurance company because you are staying in Canada longer, each new Policy will have a new effective date and you will not be covered under the new Policy for any sickness, disease or bodily injury which had manifested itself in the 180-day period immediately preceding that new effective date.
